This survey tool includes common household demographic questions and Bureau for Humantiarian Assistance (BHA)-required indicators commonly measured on humanitarian activities. To use this tool to develop a survey instrument for your impact evaluation: 

  1. Download the Excel file.
  2. Review and edit the content in column C to fit the needs of your survey. 
  3. Upload to an ODK-based survey platform. 
  4. Publish your survey and begin collecting data.
